Child Health Plus Renewal Information

If you have Child Health Plus:

• Call Health Plus at  1-800-784-7842 to set up an appointment
  or
• Go to a Health Plus Office.

If your children are eligible for Child Health Plus B, you MUST provide ONE of the following:

• Documentation of your family’s current income OR
• A Social Security Number for each household member whose income is available to the   children

Additional documentation may be required if your child qualifies for a different health insurance program offered by Health Plus. That’s why it’s best to call us before going to a Health Plus office. Our representative will review the Documentation Checklist with you, so you know what to bring. He or she will also let you know if you are required to pay a premium. 

You may also renew by mail. Call 1-800-784-7842 to request an application. The representative will assist you in completing the right application over the phone. You will have to sign the application and provide any needed documents.
